James T. is a 73-year-old man who was diagnosed with prostate cancer 3.5 years ago. Today’s telemedicine visit is a follow-up where he complains of tiredness and fatigue. Three and a half years ago, he presented to his PCP with a 2-month history of urinary symptoms (retention, increased urination at night), an elevated prostate specific antigen level (indicating the possibility of prostate cancer), and a palpable mass detected on digital rectal examination. Based on these findings, he was diagnosed with prostate cancer which was confined to the prostate and had not spread anywhere else. So his entire prostate and the lymph nodes of the surrounding region were removed. Following surgery, levels of prostate specific antigen remained undetectable during routine follow-up visits (indicating that the cancer was in remission). However, 1 year ago, his prostate specific antigen level increased. CT scan and bone scan showed no evidence of spread of the cancer. Based on these findings, he was diagnosed with recurrence of his cancer which was localized to the prostate. He was then started on treatment with androgen deprivation therapy with a drug called leuprolide which was given monthly for 6 months. He was administered EBRT (a kind of radiotherapy) to the prostate bed. At his last visit 5 months ago, prostate specific antigen levels had come back to normal. Today, he is otherwise in good health but complains of tiredness and fatigue. James T. has no relevant family history. He is married and has 3 children. He worked as a mechanical engineer and retired about 7 years ago. He has a 35-pack-year smoking history, quit 7 years ago, and no longer smokes. He consumes 1-2 alcoholic beverages per week and does not use illicit or recreational drugs.
